On naturally aspirated motors non turbo we will typically coat the inside of headers as deep as possible.

The ceramic coating is applied evenly on both the inside and outside of the exhaust pipes preventing hot spots that can cause metal fatigue improves the laminar flow of your exhaust gasses prevents rust and corrosion of the ceramic coated parts.
However if we think the coating will interfere with the fit of the header to the motor or the header to the mating exhaust part we will typically mask off those surfaces.
If the manifold or pipe has been coated not only on the outside but the inside as well it can make it almost impossible to repair since it is very difficult to get the coating inside off to.
Even if you bought titanium or good stainless steel parts a ceramic coating for headers exhaust pipes and turbo parts just to name a few actually improves their performance.

It would be a real pain to have to take the downpipe off again and have it shipped off and back again just to be coated.
